In order to replace or update your c-card,
please provide the following:

Telephone number

Social Security Number

Certifying instructor
-
Year of certification
-
City and state of certification
-
$25 payable to YMCA of the USA by check
or credit card authorization.

Scuba Diver Certification Card Replacement Form
-
YMCA accepts MasterCard and VISA credit
cards.
Note: It is not necessary to submit a
photo as there are no longer photos on YMCA
certification cards.

Upon receipt of the above information, the
certifications department usually takes one week to verify
the certification, process the paperwork, make the new card
